Just before the load drops you hear a air release. The Brake and clutch systems are operated by air on the Transi Lifts, to the best of my knowledge. Anyone ever been around one to know if that volume of air is indicative of anything such as accidentally putting it into free fall? Or are all brake release and clutch engagements actions that loud and long?

I believe by the whining sound the torque converter died in the Transi Lift forther evidence of this is the explosion caosed by the overheating and complete distruction that caused said explosion theconverterd jusr flew apart from the strain possibly from not enough throttle applied or a mechanical problem.Much of the carnage could have been avoided by placing the Transi Lift to the side of the boom being raised or lowerd. The transi lift directly in line with the large boom is what is called a “Suicide”lift for obious reasons . putting the L ampson to the side of the large boom would have put it out of harms way. Poor planning contribuited to this mishap greatly. My 30 plus years as a heavy crane operator suggest this theory to me maybe Im wrong but it makes perfict sense to me

The whining sound heard, I believe to the the sound of the hoist drum and rope spooling off at high speed. The convertor would certainly stop the drum from being powered, but the brakes on the drum should have held the load regardless of the condition of the convertor.
It is quite possible that the hoist brake was either released or failed. The resultant speed of descent would have made it impossible for the operator to catch the load by re-applying the brake-assuming that there was still as brake to employ.
